The Institute for Clinical Systems Improvement recommends that opioid
rotation, especially for patients with terminal cancer, should be considered when opioid
side effects are difficult to manage or if inadequate analgesia is present. If symptoms of
delirium and confusion are present and are attributable to opioids, a switch to a
different opioid may be advisable.
